Our plan includes
Cosmetic procedures including bleaching and veneers, night guards; all preventative care including hygiene, x-rays, exams and sealants on children and adults. It can work with flex spending. It has a high yearly maximum There is no pre-existing condition clause (If your dental condition started before we meet, we will still help) Kids may use parent's plan until out of college. (up to age 24)


Plan Features

Plan includes

  • Preventative care including two hygiene visits / year (any time) two fluoride applications / year (up to age 17) any x-rays and all exams.
  • 50% fee reduction for periodontal patients requiring more than two visits, on any additional visits (up to four total visits per year)
  • 50%fee reduction for preventive sealants for children and adults
  • 50% fee reduction on night guards and occlusal adjustments (often used to correct facial joint and muscle pain)
  • 50% fee reduction on basic cosmetic procedures including bonding.
  • 50% fee reduction on major cosmetic procedures including all porcelain crowns and veneers.
  • 50%reduction on crowns, bridges and dentures.
  • 50% fee reduction on root canals.
  • 50%fee reduction on at home bleach procedures (not all patients are candidates for bleaching)
  • 35% fee reduction on Implants (restorations only not the surgical aspect)
  • 35% fee reduction on Zoom in office bleach procedure (not all patients are candidates for bleaching)

Fees for our plan are as follows:

  • Family (up to two adults and two children) $1400.00 if paid upfront or $1600.00 if paid in three payments $175 for each additional child (up to three) until finished with college (up to age 24).
  • (Family members refer to patient's children or children under their guardianship) Individuals $750

Plan Maximum $1500 per year

Prices are for one year beginning the first day the plan is initiated. Balance of plan fees must be paid by the end of the third quarter. The amount of the maximum benefit made available will be in proportion to the percentage of the yearly fee paid to date. Patient portion of fees are due at the completion of treatment. Individual arrangements can be made for major work.

During the course of major treatment, it is sometimes necessary to call upon a specialist to consult with, or to provide supplemental treatment. Our plan provides NO coverage for these specialists. Patients will be referred at the discretion of the treating dentist; however, here is a brief description of the dental specialties and what you may expect from our practice concerning them: ENDODONTICS-This specialty involves root canals. We do the vast majority of all root canals at our office. It is possible, however that a rare referral will be necessary. ORAL SURGERY-This involves extractions, implant placement and oral pathology. We perform simple extractions at our office. Wisdom tooth removal, implant placement and oral pathology will ALWAYS be referred. ORTHODONTICS-We refer all orthodontic cases. Braces are not included in the plan. PERlODONTICS-This involves gum health. We perform non-surgical periodontal treatment. If surgery is required we refer. PEDODONTICS-This involves children exclusively. Every reasonable attempt will be made to treat all children, however, if a child is unable to cooperate and hinders their own treatment we will need to consider referral.

Benefits will not be rolled from year to year. As per our office policy a fee will be charged if appointments are not canceled within 24 hours.
